Barker and McConnell compared the efficiency of voluntary reports by directly observing some nurses as error detection methods. Thirty-six errors were documented by these reports during the year they studied those nurses. By comparison, two weeks worth of records assembled by thorough observation when extrapolated over the same one-year duration indicated that 51,200 errors have been occurred (including 600 wrong time errors).
